Updated physicals should be turned in to the school nurse.  A current physical must be on file with the school nurse in order to travel.  Physicals are valid for 13 months.  You may fax them to:  508-961-1620 or email them to the nurse contacts below.

HAZING

Massachusetts Hazing Law

The hazing of any student is prohibited by Dartmouth High School and Massachusetts State Law. Students will sign off on this policy statement (online). Any hazing related issues should be reported immediately to the Director of Music.

CORI

All CORI forms must be complete and dropped off IN PERSON with a valid driver's license for verification purposes.  For your convenience, you may drop off this form at DMS or DHS prior to 2:15 p.m. or you may bring it to 8 Bush Street before 3:45 p.m.
